APPOINTMENTS/SCHEDULING Please call for a free five minute no
obligation consultation to determine if I can help you with your
pharmaceutical needs. At that time, we
can schedule an appointment at my office or in your home, whichever is more
convenient for the patient. For a complete assessment, I will mail a
form to be completed and brought to the appointment. Alternatively, I can bring the form and
fill it out at the time of a home visit.
The complete assessment usually takes ninety minutes. One follow-up appointment with action plan
(usually thirty minutes) is included in the initial assessment fee. |
